The Customer Change Order Form is no longer in use. Please use the new Online Ordering System. If you have not setup your account yet, please follow these instructions to activate your account.

Go to Login Page (I'm Already Activated)

Activate My Account

To setup your online account:

1. Locate your Customer Number on your monthly statement.
   
2. Click ACTIVATE MY ACCOUNT.
   
3. Enter your Customer Number (located on your monthly statement, see example to the right).
   
4. Enter your last name.
   
5. Click Next. This takes you to the INITIAL ACCOUNT SETUP page.
   
6. Continue, and enter the information requested to setup your account and password.
